Medical Weight Loss: We Start With Your Labs, Not Your Meal Plan

If you have been doing everything right and the scale has not moved, the problem is probably not willpower. A sluggish thyroid, a hormone imbalance, or insulin resistance can each drive weight gain that no amount of calorie restriction will reverse on its own. Medical weight loss in Greenville, SC, at Midtown starts by identifying what is working against you.

We run the labs first. The plan comes after. This page covers how Midtown identifies the medical cause behind your weight gain, what the care process looks like, and what is included in your $85/month membership.

Three Reasons Your Weight May Not Respond to Diet and Exercise

Most weight-loss programs skip the most important question: why isn't the weight moving? At Midtown, we start there.

Low Thyroid

When TSH, free T3, or free T4 levels fall outside a healthy range, your metabolism slows down in ways that calorie restriction cannot compensate for. You may be doing everything a nutritionist recommended, and your body simply cannot respond because the underlying thyroid issue was never treated.

Hormone Imbalance

Estrogen dominance, declining progesterone, low testosterone, and elevated cortisol all contribute to fat storage, particularly around the abdomen. These are not problems that respond to cardio. They require clinical intervention once identified through bloodwork.

Insulin Resistance

When your cells stop responding to insulin efficiently, blood sugar spikes and fat storage accelerate. This shows up in fasting glucose and HbA1c results. The National Institute of Diabetes and Digestive and Kidney Diseases notes that insulin resistance often goes undiagnosed until a provider runs the right labs.

Your nurse practitioner identifies which condition is working against you before she recommends anything. That is the difference between lab-informed weight management and a diet program that guesses.

The Process

How Medical Weight Loss Works at Midtown Primary Care

01

Full Lab Panel at the Start

Thyroid levels (TSH, free T3, free T4), hormone levels (estrogen, progesterone, testosterone), blood sugar markers (fasting glucose, HbA1c), and a complete metabolic workup. Your nurse practitioner reviews the full picture and determines which findings are driving the weight issue.

02

Lab Review Appointment

You and your nurse practitioner review the results together in a 30 to 60-minute session. She explains what she found, what it means for your weight specifically, and what treatment options make clinical sense based on the numbers.

03

Treatment Plan Built Around Your Findings

Thyroid issue, she treats the thyroid. Hormone imbalance, she addresses the hormones. Insulin resistance, she targets blood sugar regulation. For qualifying members, GLP-1 medication support in Greenville is available.

04

Ongoing Monitoring

Follow-up visits are scheduled at regular intervals to track your response and adjust the plan. These visits are included in your membership.

NOTE:

Lab work is not included in the $85/month membership fee. The full lab panel ordered at the start of weight loss care is billed separately at near-cost pricing. This must be clearly communicated to the patient before the first visit and reflected in any pricing summary on this page.

Who Medical Weight Loss at Midtown Is Right For

This is not a program for people who want accountability coaching or meal planning. It is a clinical service for adults whose weight is not responding to lifestyle changes due to an underlying medical condition.

Frequently Asked Questions

  • Is medical weight loss included in my DPC membership?

Yes. The clinical evaluation, lab reviews, treatment plan visits, and ongoing monitoring are all included in your $85/month. The only separate cost is the medication itself, which you fill at a pharmacy at the standard pharmacy price.

  • Does Midtown offer GLP-1 medication support?

Yes. GLP-1 medication support is available for members who qualify clinically. Your nurse practitioner evaluates your labs and health history before prescribing. The medication is filled at a pharmacy and billed at the pharmacy price. Midtown does not mark up prescriptions.

  • What labs do you run before starting a weight loss plan?

Thyroid panel (TSH, free T3, free T4), hormone levels (estrogen, progesterone, testosterone), fasting glucose, HbA1c, and a full metabolic workup. These are the labs that identify the medical reasons behind treatment-resistant weight gain.

  • How is this different from a program like Noom or Weight Watchers?

Noom and Weight Watchers are behavioral programs built around accountability and food logging. They do not run labs. They do not identify thyroid dysfunction, insulin resistance, or hormone imbalance. If a medical condition is driving your weight gain, a behavioral program will not resolve it. Midtown starts with the medical cause.

  • What if I have tried GLP-1 before and it did not work?

Your nurse practitioner reviews your full history before recommending GLP-1. If a previous attempt did not produce results, she examines what drove the resistance: the dose, underlying thyroid function, concurrent hormone issues, or dietary patterns that counteracted the medication's effect. The plan is built around your current lab results.

Ready to Find Out What Is Actually Stopping Your Weight Loss?

If you have spent years doing the right things and getting nowhere, the answer is not more discipline. It is the right diagnosis. Medical weight loss in Greenville, SC, at Midtown starts with the labs your previous provider never ordered, finds what is actually working against you, and builds a plan around that, not around a generic calorie target that ignores your thyroid, your hormones, and your metabolic reality.

The scale is not your enemy. The missing diagnosis is.
